Responsibilities Coordinates and oversees all aspects of patients' home care. Collaborates with patient care teams to develop and implement care plans. Other duties as assigned. Qualifications One year of clinical experience as a registered nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP clinical. Current RN license, specific to the state(s) you are assigned to work. Minimum six months home health experience. Current CPR certification. Valid driver's license, reliable transportation and liability insurance. Able to assess patient status and identify requirements relative to age specific needs.
