How you'll make a difference… In this role, some shifts you will have a lot of fun! There will be games, laughing, learning, and adapting. Some may challenge your abilities to critically think and problem solve as a Licensed Practical Nurse. At the end of each shift, you will feel the reward and pride in the services and support you provide that allow clients and families to live a joyful life together at home! What you'll be doing… Our Registered Nurses and Licensed Practical Nurses work in our clients' home providing skilled medical care. Schedules are based on the client and the family needs. No two clients are alike. Key responsibilities include but are not limited to: Complete assessments, medication administration, and provide therapies, treatments, and interventions as scheduled in accordance with the plan of care and physician’s orders. Partner with all members of the interdisciplinary team including the family, ensuring a constant flow of communication regarding the client’s cares, changes in condition, developmental goals, etc. Provide support and education to clients and families to help them be successful. Build rapport and trust with clients, families or other caregivers, the office team, and peers to facilitate effective communication and build relationships. Complete timely charting through our electronic medical record system. Keep up to date with the client’s plan of care, changes in conditions, physician’s orders, family, or caregiver requests, etc. Advocate for the client and family to promote and support growth and development. Examples of some of our nursing cares include gastrostomy tubes, jejunostomy tubes, nasogastric tubes, PICCs, PORTs, tracheostomies, seizure disorders, etc. What’s required? Active Registered Nurse or Licensed Practical Nurse (LPN) licensure, without restrictions Valid driver’s license and proof of auto liability insurance coverage CPR certification required but may be obtained after hire COVID-19 vaccine compliance Ability to lift, carry, push, or pull up to 50 pounds Who we are looking for?
