Responsibilities: About the Role During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers: Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols Administer and order point of care and lab-based testing as needed Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities Benefits of Working at Matrix Flexibility – Visits are scheduled based on YOUR availability. We conduct assessments Sunday – Saturday from 8 am – 7 pm. You can work during the day, evening, and/or weekend based on your schedule. Competitive Pay – Earn income based on the number of assessments you complete. Increase your earning potential by offering greater availability and traveling to see members outside your home location. Those completing a monthly average of 20 – 100 visits earn $1,800 – $11,500 per month. This is based on the per assessment completion rate range of $90 – $115 depending on your state of residence. In addition to comprehensive onboarding training we will assign you a preceptor, provide 1:1 feedback, and offer free access to earn continuing education. Qualifications: Skills & Experience That Shine Master’s Degree or commensurate experience and satisfactory completion of NP licensure Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty Current BLS, ACLS or CPR certification 1 year of NP experience preferred, new grads encouraged to apply! Position Summary Eventus WholeHealth delivers an integrated model of care to adults who reside in skilled nursing and assisted living facilities through a network of healthcare providers including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants and in-house Support Staff. Essential Duties And Responsibilities Provide primary-care services in accordance with Eventus WholeHealth site-specific protocols and consistent with the standard of care for the specialty of family medicine, geriatric medicine, and/or internal medicine as it pertains to long term care facility work. Quality/Compliance Review and sign MAR’s as requested by facilities, sign all orders specific for Primary Care while at the facility and/or within 24 hours if faxed or emailed to include phone orders, DME orders, Death Certificates, Plans of Care, PT/OT orders, hospice orders as appropriate, and other orders as is relevant and appropriate to the facility patient care needs and Eventus WholeHealth. Review and address labs within 24 hours of them being sent to you. Manage all narcotic prescriptions specific to your facility’s patients and as per Eventus WholeHealth Narcotic Rx Policy. Provide facility with adequate time each visit to allow for paperwork administrative duties. Provide on-call triage duties as per the on-call triage protocol policy. Triage Policies and on-call duties and scheduling will be modified as needed and all providers are expected to adhere to the newest agreed upon triage schedule. Eventus WholeHealth triage policies will be based upon a majority vote of all involved Eventus WholeHealth providers. Complete documentation and submit billing records to Eventus WholeHealth according to the published policies on charting and bill sheet submission and provide the facility with a hard copy of the electronic documentation within 48 hours of service for inclusion in the patient’s facility medical record. Establish and maintain open and positive communications with facility staff and administration. Provide verbal and/or written instruction or feedback regarding medications and other pertinent caregiver information.
