We’re looking for a teammate who is an innate people person, loves helping others, and has a knack for tenacious problem solving.This is a part‑time (up to 24 hours/week) hybrid role. You'll spend time in the field, traveling to see patients within Aroostook County or working from an office based in Presque Isle, with some remote work from home possibilities. This is a 50% clinical and 50% administrative role with general work hours scheduled Monday through Friday from 8am‑5pm ET.Here’s what you’ll doClinicalAt patient visits, obtain and document vital signs (blood pressure, temperature, pulse, respirations, weight, height) in the EHRPerform venipuncture and capillary blood collection for laboratory testing following proper phlebotomy protocolsCollect, process, and ready any specimens for laboratory analysisAdminister immunizations and injections (as able) and per physician orders while maintaining accurate vaccine recordsAssist providers with minor procedures and examinations as neededWhen in the Field, relay pertinent environmental advantages/disadvantages that will impact the patients well being to the care team, completing physical and social screenings as assigned by the providerEducate patients on treatment plans, medications, and follow-up care as directed by providersDocument patient interactions, clinical data, and care activities accurately in the electronic health recordCarry out Pop Health care gap closures efforts and sprints as needed per payer contractEnsure proper sterilization and maintenance of medical equipmentFollow infection control protocols and maintain compliance with safety standardsBe available for field and urgent deployment within your market region even on unplanned field daysConnect folks with the various Galileo services and teams, including clinical and administrative functions.AdminBe a local “face” of Galileo, representing the organization professionally, embodying our mission, and creating meaningful, lasting relationships with all stakeholdersOwn overall empaneled patient chart management, prepping charts medically and demographically to ensure readiness and thoroughness of care can be performed smoothly and efficientlyRun-The-List in partnership as a care team to ensure all orders, care coordination and constant communication is seamless across each empaneled patients care planMaintain exam room, mobile unit and/or office cleanliness, organization, and adequate supply levelsManage inventory of clinical supplies and medications, placing orders as neededRoute inbound mail appropriately, sending patient letters or medical equipment as neededCoordinate with insurances to submit and complete prior authorizations and referralsRequest, follow up and file medical records with local hospital systems, PCPs and area HIESSubmit Pop Health documents to maximize compliance with payer contracts as neededCommunicate with patients via text, video or phone as needed for care coordination, scheduling, and reactive needsParticipate in projects and events as assigned by the Director of OperationsAbout YouYou are a creative problem solver and a compassionate provider of support.
