This role involves clinic-based care with occasional home visits and telehealth support. The NP will manage chronic conditions, develop treatment plans, educate patients, collaborate with an interdisciplinary team, and maintain thorough documentation aligned with risk-adjustment expectations.Key ResponsibilitiesConduct patient assessments, physical exams, and diagnosticsManage chronic conditions (e.g., diabetes, COPD, CHF, dementia, debility)Develop individualized treatment plans aligned with patient goalsEducate patients and families on medications, nutrition, disease progression, and preventionCollaborate with interdisciplinary team members to coordinate carePrescribe medications within state scopeProvide care in clinic, patient homes, and via telehealth as neededRequirementsMasters degree in NursingBoard-certified NP (Adult, Adult-Gero, or Family)2+ years Primary Care outpatient experience requiredExperience with value-based care (preferred)Must be able to practice autonomouslyNo new gradsActive NP license (state-specific)DEA (or eligibility)Experience with geriatrics, chronic disease management, wound care, injections, ABIs, or imaging interpretation (preferred)EMR experience; Athena highly preferredBenefitsCompetitive salary + bonusMedical, dental, vision, 401kPTO + parental leaveCME reimbursementState license reimbursementSupportive, team-based environmentGrowth and leadership opportunitiesExcellent work-life balance (weekday schedule)For More Information contact
