In this role, you will assess, plan, organize, and participate in rehabilitative programs that help restore vocational, homemaking, and daily living skills, as well as general independence, to disabled persons. Perform initial and ongoing physical therapy evaluations according to APTA standards and practices. Develop treatment plans by identifying a problem list, including short and long-term goals and methods to achieve identified goals. Implement and coordinate the treatment plan, modify it as necessary, and monitor progress. Educate patients and their families about treatment plans/schedules and self-exercises to be continued at home. Document patient care services by charting in patient and department records. Coordinate with other healthcare professionals to provide holistic care to patients. Proficiency in physical therapy assessment and treatment Strong interpersonal and communication skills Ability to manage multiple patient caseloads effectively Proficiency in using physical therapy software and equipment Medical Insurance Dental Insurance Vision Insurance 401k Current state license as a Registered Physical Therapist / PT Valid driver’s license and reliable transportation CPR certified
