Our office is in Roseville, MN and the service area of where our patients live is in the 11 County Twin Cities Metro Area. The general territory for this position would be the Northwest Metro area. License is active and in good standing. At least 1200 hours of experience as an LPN/LVN or completion of the Office's preceptor program. 2 years experience as an LPN is preferred. Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance. CPR Certification Some functions of what you would do as an LPN: Participate in ongoing interdisciplinary assessment of the patient. Participate in the development and evaluation of the plan of care in partnership with the patient, representative, and caregiver(s). Provides services that are ordered by the physician as indicated in the plan of care. Recognizes and reports changes in the patient’s condition to the RN. Counsels and educates the patient and family in meeting healthcare related needs. Completes timely and accurate clinical notes including addressing patient’s progress. Job Description: We are seeking dedicated, patient-focused physicians licensed in at least to join our team. Whether your background is in primary care, internal medicine, addiction medicine, emergency medicine, or urgent care, this role provides an opportunity to practice modern, high-quality medicine without the constraints of a traditional practice setting. While most of our services are fully virtual, in-person hybrid opportunities may be available. Responsibilities: Provide exceptional virtual care, including patient assessments, medical history reviews, and appropriate medical treatment. Deliver compassionate, patient-centered care, building therapeutic doctor-patient relationships. Order and interpret lab tests as needed to make informed, high-quality treatment decisions.
