It is essential that we work well together to ensure the proper delivery of quality care. Each employee must remember that our primary objective is to meet the needs of our residents. We are here to serve them. The following job description is designed to assist you in becoming part of our team. I. JOB SUMMARY: A. The Licensed Practical Nurse (LPN) works in cooperation with the Registered Nurse to provide high quality health care according to the resident's needs and in conformance with recognized nursing techniques, procedures and administrative policies. The LPN is responsible for their own actions relating to the nursing care furnished to residents and cannot avoid the responsibility by accepting the orders or directions of another person. II. PERFORMANCE REQUIREMENTS: A. RESPONSIBILTY FOR: Performance of any acts in the care, treatment and observation of the resident. Maintenance of the health of others and promotion of health care for our residents. Assisting with health counseling and teaching. Applying procedures to safeguard life and health of residents including the administration of medications and treatment consistent with the Practical Nurse education and preparation under the direction of a physician, dentist or registered nurse. Maintaining confidentiality. B. PHYSICAL DEMANDS: Good mental and physical health. Good finger and hand dexterity required to handle and manipulate instruments and equipment. Visual and aural acuity to detect changes in the resident's condition. Good physical agility to stand, bend, sit, walk, stoop and lift up to between 51 and 74 pounds. C. SPECIFIC DEMANDS: Ability to recognize there may be: *exposure to communicable diseases * experience of muscle strains due to lifting or moving residents or injury from irrational patients. * possibility of cuts or minor burns from instruments or equipment III. QUALIFICATIONS A. EDUCATION: Graduation from an accredited school of Licensed Practical Nursing and a license to practice as a Licensed Practical Nurse in the State of South Dakota. Is certified in Basic Life Support. B. JOB TRAINING AND EXPERIENCE:Experience as an LPN in health care setting, preferably experience in a nursing home setting. IV. WORKING ENVIRONMENT: A. Works indoors in well lighted and ventilated nursing home rooms. V. WORK PREFORMED: A. Practices as a Charge Nurse and organizes total nursing care when in charge. B. Delegates nursing tasks but is responsible and accountable for the completion of the tasks and the quality of care given. C. Collaborates with the Registered Nurse in each step of the nursing process: assessment, nursing diagnosis, plan of care, implementation and evaluation. D. Is knowledgeable of resident's condition at all times and reports pertinent changes to the family, physician and Director of Nursing. E. Documents in the medical record information that reflects any change in the condition or behavior of the resident. F. Provides health teaching to the resident or family based on health related problems or needs and documents response to teaching on the medical record. G. Demonstrates skills in nursing functions such as catheterization, sterile irrigation, giving oral and parenteral medication (intramuscular and subcutaneous injections) and other functions that are within the basic LPN scope of practice. H. Effectively uses appropriate communication principles with patients, families and other health care professionals. I. Uses appropriate designated communication channels to identify and discuss resident or staff problems, issues and concerns. J. Orders medication from the pharmacy according to the facility procedure. K. Attends staff meetings, supervisor meetings, pharmacy meetings, special workshop approved by administration. When unable to attend reads, signs and dates the minutes of the meetings. L. Ensures safe therapeutic environment by seeing that all equipment used is in correct functioning order and/or by identifying the need for repair, replacement or evaluation of faulty equipment by the appropriate personnel. M. Documents and reports all variances which include medication errors. N. Knows and follows established procedures for infection control, safety and fire and disaster evacuation plans, and has read these manuals as well as the personnel policy manual. O. Assures a safe therapeutic environment by seeing that all equipment is used in correct functioning order and/or by identifying the need for repair, replacement or evaluation of faulty equipment by the appropriate personnel. P. Is personally clean and neat. Q. Performs other duties as assigned. We do have an opening in our Traumatic Brain Injury Unit which is an 8 bed rehab unit and an opening in our regular skilled nursing home unit. We are offering a $5000 sign-on bonus for a 2 year commitment. The sign-on bonus is paid over a 2 year period of time.I. JOB SUMMARY: A. Demonstrates skills and nursing functions such as starting I.V.'s, giving oral and parental medications, and performing treatments. P. Assures a safe therapeutic environment by seeing that all equipment is used in correct functioning order and/or by identifying the need for repair, replacement or evaluation of faulty equipment by the appropriate personnel R.
