They provide comprehensive public health services including general medical, surgical follow-up, pediatric, prenatal and postpartum care. Facility Address: 6822 E 1000 S, Fort Duchesne, UT 84026 Physician Rheumatology - RESPONSIBILITIES (listed are included but not limited to) The rheumatologist physician shall interview and examine patients, review past medical history, and request and slash or perform diagnostic tests and examinations deemed necessary to obtain all possible information related to each case. Will provide definitive management for rheumatologic outpatient practice within the limitations of the clinic. Shall conduct outpatient clinics making observations appropriate to the outpatients underlying disease and treatment plan including current symptoms. Review the patient's medication diet therapy course adherence to management plan and patient observations modify regiment as indicated. And struck and counseled patience and family members as required and does so in a culturally sensitive manner. The rheumatologist shall refer patients to appropriate contract in medical facilities, providing full clinical information for care and diagnostic procedures that cannot be adequately provided at the Uintah ad Ouray Service Unit. Shall assure preparation of appropriate medical records for all patients seen to assure the accumulation and organization of all pertinent clinical data needed to provide comprehensive medical care. Will work closely with allied health professionals to utilize and coordinate the services of other health care professionals in the management of the patient's medical, psychological and social problems. Will provide prenatal care to expectant patients. Will efficiently navigate via electronic Medical record for a prescribing, documentation, and orders including referrals in compliance. Will perform diagnostic and therapeutic procedures and diagnostics normally performed by a rheumatologist. Rheumatologist Physician - REQUIREMENTS Experience: Must have a minimum of two years' experience as a Rheumatologist. Certification: Board certification is preferred within Rheumatology. Education: Minimum education is a doctorate degree in Osteopathy or Allopathic medicine, along with completion of an internal medicine residency program, as well as a fellowship completed in rheumatology. Licensure: The physician must have an active, unrestricted, and free of disciplinary action license as a physician in a State, District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States. This will include obtaining and documenting in the EHR in ophthalmic and medical history, a comprehensive ophthalmic exam, accurate and complete diagnosis, and effective treatment plans. Treatment plans will include (as clinically indicated) a prescription of eye care, contact lenses, oral and ophthalmic medications, and ordering ancillary testing as needed. Ancillary testing may include but not be limited to obsolic imaging studies (ultrasound, photos, OCT, pachymetry, Etcetera), visual field testing, laboratories studies, and radiographic imaging (playing films, CT, and MRI). Minor surgical procedures such as foreign body removal and papilloma excision, and pre-operative and post-operative care of various ophthalmic surgeries, is also expected. In all cases documenting, performing, and arranging follow up and referrals as needed is required as well. Optometrist - REQUIREMENTS Experience: A minimum of 2 years' experience as a clinical optometrist. Education: Must have a Doctor of Optometry degree. Licensure: Must have an active, unrestricted, and free of disciplinary action license as a nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a Territory of the United States. Certification: Must have a current certification in Basic Life Support.
