Overview
The Hospice Team Physician provides compassionate, patient-centered care to individuals with life-limiting illnesses. The role focuses on symptom management, emotional support, and coordination of care to ensure comfort and dignity during the end-of-life process.
Job Responsibilities
Provides clinical care for hospice patients across all levels of care (General Inpatient, Continuous, Routine and Respite) in all settings of care, including inpatient hospitals, senior living communities and private homes.
Participates as a member of the interdisciplinary team to assure that interdisciplinary team functions are performed, including the development and implementation of written patient care plans with goals of care and individualized treatment plans.
Manages pain and other distressing symptoms using appropriate pharmacologic and non-pharmacologic interventions.
Conducts face-to-face assessments to determine hospice eligibility and ongoing appropriateness of care.
Collaborates with interdisciplinary teams including nurses, social workers, chaplains, and volunteers.
Provides guidance on advance care planning and end-of-life decision-making.
Maintains accurate and confidential patient records.
Ensures compliance with state and federal hospice regulations.
Works with the interdisciplinary team of nurses, social workers, chaplains, and other healthcare professionals.
Participates in weekly team meetings to review patient care plans.
Acts as a liaison between the primary physician and hospice team.
Provides emotional and psychological support to patients and families.
Guides families through end-of-life decision-making, including advance care planning.
Communicates clearly and compassionately about prognosis and treatment options.
Participates in undergraduate and graduate medical education, and with other learners as determined by medical leadership.
Participates in weekend bedside duty and nighttime on-call activities as part of an equitable on-call schedule.
Qualifications & Requirements
Education: Graduate of an accredited school of medicine or osteopathy (MD or DO). Completion of an approved residency. Board Certified in Geriatric Medicine or Hospice & Palliative Medicine. Qualified but non-board-certified physicians with extensive experience may be considered.
Certification/Licensure: Current Virginia State license (or ability to obtain), DEA, and CDS certificates. Eligible for Inova Medical Staff membership.
Payer Credentialing: Ability to complete credentialing with third-party payors and enrollment as a CMS Medicare/Medicaid payor.
Medical Staff Credentialing: Successful completion of credentialing for Medical Staff membership and appropriate clinical privileges.
Preferred Qualifications
At least 2-3 years of direct relevant clinical care experience, preferably in general inpatient, senior living, and home hospice settings.
Experience in multidisciplinary teams caring for at-risk patients with advanced age or end-of-life circumstances.
Demonstrated leadership and collaboration skills, with the ability to work with diverse healthcare and community services.
Proficiency in using computers and a variety of productivity applications; strong oral and written communication skills; data analysis and interpretation capabilities.
Ability to develop goals, objectives, critical paths, systems, new programs, and policies and procedures.
