Job Description
As a therapist at CenterWell Home Health, reporting to the Clinical Manager, you will help patients regain strength, mobility and independence from the comfort of their homes by delivering personalized, functional‑focused care that empowers individuals to overcome limitations and improve their quality of life.
Responsibilities
Plan and administer prescribed physical therapy treatments in patients’ homes to achieve the highest level of physical function.
Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
Develop and implement rehabilitation programs consistent with the physician’s Plan of Treatment and the goals of the patient team.
Provide PTAs and Home Health Aides with clear written care instructions and supervise their performance and documentation.
Document patient observations, interventions, and evaluations promptly and thoroughly.
Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
Required Qualifications
Current and unrestricted Physical Therapy license.
Current CPR certification.
Experience collaborating with a team of healthcare professionals.
Preferred Qualifications
One year of experience as a Physical Therapist.
Pay Range
$59.00 - $83.00 per visit/unit and $93,000 - $128,000 per year base pay.
Working Conditions
Patient‑facing role subject to Humana’s Tuberculosis (TB) screening program.
Driver safety program requires a valid state driver’s license and personal vehicle liability insurance as specified by state minimums.
Scheduled weekly hours: 40 hours per week.
