Job Summary The Licensed Practical Nurse (LPN) provides general nursing care to patients, families and caregivers in the Medical Group practices. The Licensed Practical Nurse (LPN) assists and advocates for the patient and families health in support of the patient centered approach and delivery model of care, and is an active care team member. Responsible for collaborating with physicians/ providers , registered professional nurses and multidisciplinary care team members. A Licensed Practical Nurse (LPN) can perform tasks and responsibilities within the framework of case finding, health coaching, and supportive care under the direction of the registered professional nurse or licensed physician or licensed health care provider. Nursing shall perform in accordance with the standards outlined for Licensed Practical Nurse (LPN) in the NYS Education Law, NYS Office of the Professions, and by the State Nurse Practice Act. A Licensed Practical Nurse will follow the mission, vision, I-CARE values, policies and procedures of Mohawk Valley Health System. Core Job Responsibilities Gather patient measurements and collect patient data Record and report patient findings Prepare patient for patient examination and treatment Assistswith examination and treatments Perform tasks within the framework of supportive care, under the clinical direction of licensed health care provider or registered professional nurse Support self-care , self- management and the established plan of care. Ensure clinical policies and procedures comply with current state and federal guidelines. Ability to identify and recommend solutions to procedural issues/opportunities and works directly with the Office Manager to resolve. Recommendations are to ensure quality, compliance, safety and cost efficiency. Ensure and maintain adequate clinical supplies, medication storage and PAR level management. Ensures clinical equipment remains in operation condition. Ensure emergency equipmentlmeds remain up to date and in appropriate operating condition. Attend office staff meetings and relay pertinent information to clinical team members timely. Atterid periodic training sessions and meetings pertinent to position. Perform all duties as nursing support to assigned provider or office; continues in support role. The Work Appropriately codes medical information, correctly applying current coding guidelines.
