The law requires us to provide equal employment opportunity to qualified people with disabilities. We have a goal of having at least 7% of our workers as people with disabilities. The law says we must measure our progress towards this goal. To do this, we must ask applicants and employees if they have a disability or have ever had one. People can become disabled, so we need to ask this question at least every five years. Completing this form is voluntary, and we hope that you will choose to do so. Your answer is confidential. No one who makes hiring decisions will see it. Your decision to complete the form and your answer will not harm you in any way. Locally owned and operated agencies in more than 1,000 communities throughout North America I worked at Home Helpers of Bourbonnais for 5 year and it was by far the best job I’ve had. The owner is very invested in her business and truly cares about her staff and Clients. The office staff is very professional and does a great job managing. As a caregiver you can have as many or as little hours as you wish. Schedules do change but that’s only when the clients need changed. I also had an updated care plan for every client and I was able to assist with making sure that my clients had the best care as my shift ended. Supervisors are not at every clients house all the time but they are very involved and always reachable. #J-18808-Ljbffr
