BestSelf's ACT Brave program is designed to address the significant needs of youth ages 10 up to 21, who are at risk of entering or returning home from high intensity services. Youth ACT (Assertive Community Treatment) serves as a critical component in the children's continuum of care, ensuring the child and their family have the necessary support services and access to clinical professionals. Youth ACT teams deliver intensive, highly coordinated, individualized services through an integrated, multi-disciplinary team approach to maintain the child in the home, school, and community.

SUMMARY OF DUTIES

Family Peer Advocacy Services are provided to families raising a child or young adult up to age 21 who is experiencing social, emotional, addiction, or behavioral health challenges. Family Peer support services are delivered by a trained Family Peer Advocate (FPA) who supports the parent/caregiver to enhance their skills and promote positive youth functioning.
