Job description Wise Medical Staffing & Home Care is expanding into the Central Ohio region. We are a respected and established healthcare agency in South Eastern Ohio and Dayton, and we are excited to begin to offer home care services to the patients in this area. We are seeking multiple experienced and compassionate Licensed Practical Nurses (LPNs) to join our team of home health professionals. We have full, part time and prn positions available and our Central Ohio Region serves the following counties: Franklin, Delaware, Fairfield, Pickaway, Licking, Madison, Union, Morrow, Knox, Hocking, Perry and Fayette Counties.
You may work an entire shift in a single patient’s home, or your shift may consist of traveling between multiple patients’ homes for shorter periods of time.
Responsibilities of an LPN doing Home Health
Is able to understand, adhere and follow established policies and procedures in patient care. Provides skilled care according to care plans established by the RN. Willing to collaborate and assist with the evaluation of the plan of care. Communicates effectively and consistently to patients, caregivers and staff. Provides supportive education on illness, medications, treatments to patients and their caregivers in a level they can understand. Takes vital signs as needed - pulse, temperature, respirations, pulse oximetry. Comfortable with working with and providing education on oxygen therapy, use of nebulizers and sleep apnea machines. Skilled in venipunctures to obtain blood samples for lab work. Provides wound care as directed by physician's orders. Able to educate caregivers to assist prn with patient's wounds. Provides care and education regarding various ostomies and trachs. Able to perform, monitor and care for urinary catheters. Documents all interventions in the electronic medical record, communicating any needs or concerns to the RN immediately. Attends staff meetings, in-services, educational events and community events as requested. Able and willing to be a team player and assist in the growth and referrals for the agency through the provision of excellent, patient-centered care. Able and willing to learn the fundamentals of health care regulations regarding home care. Willing to drive to outlying counties initially to provide patient care and expand referral basis.
Licensed Practical Nurse (LPN) Qualification:
Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state of Ohio. Minimum of one (1) year experience in nursing, required. Certified in Basic Life Support (BLS). Able and willing to work with patients in a home setting. Trustworthy, motivated and able to work individually as well as collaboratively. Owns and maintains reliable transportation and vehicle insurance.
Job Type: Full-time, Part-time and PRN/Casual. We are hiring multiple candidates in the region. Pay: $32 to $35 per hour commensurate with experience. Benefits:
