The first primary purpose of this position is in the provision of care coordination services to Maternal Health patients, for the provision of Care Management for High Risk Pregnancy (CMHRP) program for ante-partum and postpartum patients. The second primary responsibility of this position is to serve as a care manager for the Care Management for At-Risk Children (CMARC) program. Provides comprehensive management services through care coordination to pregnant and postpartum women ages 12-45 who are receiving prenatal and postpartum care in the private and public sector. Developing an individualized patient care plan within the Virtual Health system.
