$83,000‑$92,000 annually
Position Summary
The Resource Nurse completes hospice admissions and visits and is an integral part of the Resource Care Team and admission process. This role requires effective collaboration with Intake, Clinical Managers, RN Case Managers, and other interdisciplinary team members to coordinate patient care and schedules. Key responsibilities include responding to patient/family or facility requests, completing admissions, documenting patient visits in the electronic health record (EHR), obtaining physician orders, and communicating any changes in patient condition or needs.

Qualifications
Graduate of accredited school of nursing
Current Minnesota RN licensure
Three years recent nursing experience in acute, long‑term care or home care/hospice/public health
Patient care planning and patient teaching skills
Written and verbal communication skills
Critical thinker
Current Minnesota driver’s license, insurance and daily access to reliable vehicle
Ability to use MS Outlook and familiarity with EHR use on iPads and computers
Job Duties
Complete hospice admissions as scheduled
Complete follow‑up visits to patients as needed
Collaborate with clinical managers and RN CMs to coordinate schedule as necessary
Respond to incoming calls for requests for assistance from patients/families or facilities
Communicate significant findings, problems, patients’ needs, changes in condition or environment to physician, case manager and other members of the inter‑disciplinary team
Document all patient visits in EHR within specified timeframe, per company policy
Obtain physician orders as required
Enter visit schedule into clinical database software on a daily basis
Role is subject to adjustment and change to meet the needs of the hospice patients
