Position Registered Nurse (RN) - Home Health and Hospice
Setting Home Health and Hospice
Shift Full-Time
Compensation $40.00 – $48.00 per hour
Sign-On Bonus: $5,000
On-call compensation structure (availability pay plus hourly rate for visits)
Qualifications Graduate of an accredited school of nursing.
CPR certification upon hire.
Valid driver's license with reliable, insured vehicle for community travel.
Strong assessment, communication, and clinical documentation skills.
Ability to work independently and manage patient caseload effectively.
Position serves both home health and hospice patients with flexibility between service lines.
Travel throughout mountain communities required, especially challenging in winter conditions.
On-call rotation shared among nursing team.
Preferred: Bachelor's Degree in Nursing (BSN).
1-2 years of recent acute care experience in institutional setting.
1-2 years of recent home health or hospice experience.
Experience with both home health and hospice service lines.
Familiarity with rural healthcare delivery and mountain community needs.
Job Summary Provide comprehensive nursing care to patients and families in their homes across home health and hospice service lines.
Conduct initial and ongoing assessments to determine patient and family care needs.
Develop, implement, and regularly evaluate individualized care plans based on patient status and goals.
Deliver skilled nursing interventions including medication administration, wound care, and treatments per physician orders.
Serve as case manager for assigned caseload, coordinating care with interdisciplinary team members.
Educate patients and families on self-care techniques, medication management, and health maintenance.
Collaborate with physicians, therapists, and community resources to optimize patient outcomes.
Maintain accurate clinical documentation and communicate changes in patient condition to care team.
Support patients and families emotionally throughout care continuum, particularly in end-of-life care.
Participate in on-call rotation providing after-hours patient support (reimbursed at Colorado minimum wage for availability, hourly rate for patient visits).
