Job Description
PRIMARY FUNCTION
The Occupational Therapist will serve as a member of the rehabilitation and habilitation teams. They will provide evaluations and appropriate intervention within their scope of practice. This position will travel to our offices located in Oradell, Wayne, Paramus, and Hackensack, NJ. Monday - Friday (21/27 hrs/week, 3-5 days per week).
PERFORMANCE RESPONSIBILITIES
Works toward the accomplishment of department/Agency goals
Maintains confidentiality and immediately reports to management breaches in confidentiality
Observes and maintains appropriate professional boundaries with members, co-workers and supervisors
Attends all scheduled meetings as assigned
Meets relevant continuing educational/training requirements
Completes all required documentation/paperwork accurately and within established time frames
SUPERVISORY RESPONSIBILITIES, when applicable
Provides direct supervision to assigned employees in order to enhance their job skill set.
Provides on-going support to employees to assure completion of job responsibilities
Carries out supervisory responsibilities in accordance with the agency's policies, applicable laws and governmental regulatory agencies
Reviews employees' responsibilities to determine time frame and scope of authority, reviews employees' requirements and allotments of available resources
Evaluates employees' productivity and documentation of service delivery
Responsibilities include interviewing and training employees; planning, assigning and directing work appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems
ESSENTIAL FUNCTIONS
Perform assessment and evaluations upon receipt of referral/prescription from primary care MD, when required.
Develop a Treatment Plan based on the evaluation and obtain MD certification to proceed to treatment.
Plans, monitors and executes therapy according to the identified needs of the person; implements treatment programs; documents and supports treatment activities.
Delivers individual and/or group intervention as required.
Maintain contemporaneous documentation in clinical records to meet compliance standards. Submits written reports to appropriate sources according to schedule utilizing required formats.
Become expert as to the regulations, policies and procedures of the agency, and maintain compliance with DDD, Medicare, Medicaid and Third Party insurance requirements for service provision.
Designs, fabricates, adjusts or makes minor repairs to therapeutic equipment for individuals; suggests and monitors provision of appropriate assistive technology; suggests new equipment purchases, when appropriate.
Work as part of the program/vocational/clinical team and attends meetings as appropriate.
Consults regularly and as needed with rehabilitation or habilitation staff to determine treatment goals and strategies needed.
Is familiar with and adheres to all HIPAA related policies and procedures safeguarding the protected health information ("PHI") of all patients.
Oversees activities of Therapy Assistants, Therapy Aides or therapy students and cosigns all documentation according to discipline specific practice standards and licensing regulations when applicable.
Attend Life Plan meeting as needed / requested.
Trains day program and rehabilitation staff on any recommended strategies that will positively impact the habilitation or rehabilitation goals.
Conduct Staff trainings and/or presentations to the community.
Other activities as assigned by Director.
Qualifications / Specific Requirements Include
Minimum Education: B.S. / M.S. (preferred) in Occupational Therapy
Minimum Experience: 1 year in a related setting
Knowledge/Skills/Abilities (KSAs):
Knowledge:
Demonstrated knowledge of and independence in the use of varied therapy treatment approaches
Knowledge of normal development
Skills and Abilities:
Ability to respect and maintain confidentiality
Communicate effectively verbally and in writing
Perform strenuous physical activities
Travel to various locations
Engage individual / family / guardian effectively and openly in treatment planning
Valid Driver's License
Required Certification/Registration: NJ State License
