About the Department Our Post-Acute Care Transitions (PACT) Program is an award-winning initiative which focuses on patients who are medically stable and require nursing home level of care, but who do not yet have a safe discharge plan after hospitalization. Using a multidisciplinary approach, PACT coordinates a seamless transition for patients from the hospital to the nursing home where patients can access resources related to mental health, homeless medicine, substance use, wound care, housing programs, and palliative services. Once the patient is ready for discharge from the contracted nursing home, a team-based assessment informs a care coordination plan for patient integration back into the community. About the Position The Post-Acute Care Transitions Program is seeking a physician join our established team. You should have a current CA medical license, DEA and eligibility to participate in Medi-Cal/Medicare. The successful candidate will work closely with our robust interdisciplinary team to provide clinical care to our diverse, fragile patients, as well as coordinate transfers from the hospital, to the contracted SNF and facilitate safe discharges to the community. There is also opportunity for QI projects for the division as well as teaching Internal Medicine residents. This position also offers full time employment to work half time in Post-Acute Care Transitions and half time in Primary Care clinic. Job Description Job Description The Set Up Technician must be self-motivated in the installation of molds, operation of the molding machines and all support equipment as well as any miscellaneous tasks directed by the Molding & Validations Manager

DUTIES AND RESPONSIBILITIES • Daily turnovers for effective communication with molding engineer when person is on site or thru detailed e-mail if face to face communication is not possible. • Safe mold handling • Material handling, mixing, drying, analysis, inventory & documentation • Mold interface hookups, water, electrical, clamping, knockouts, sprue, misc. to the molding machines is complete, & documented • Participate in sampling reviews as directed by molding engineer. • Production molding run setup & startup • Operate molding machines and all support equipment as necessary to complete production runs on designated shift as required. • Maintain the system for tagging/cleaning/storage of all molds • Maintain molding support equipment • Maintain Raw material storage system • Conduct Housekeeping as required • Assist with shipping/receiving preparation as needed

MINIMUM REQUIREMENTS AND QUALIFICATIONS • This position requires the person to be self-motivated and be able to work independently to planned tasks • Accurate, consistent, & documented mold installations as directed by the molding engineer • Product quality verification per Quality Manager to meet the customer’s expectations • Maintain the cleanliness of both the plant and equipment as directed by the Molding & validations Manager • E-2 Shop System • Planned, documented mold tryouts & process development system • Performance Metrics training / goals • Must be able to lift and move up to 100lbs. infrequently • High School Graduate, advanced degree a plus • Good Mechanical aptitude required • Ability to run all molding machines and support equipment

ESSENTIAL FUNCTIONS AND PHYSICAL DEMANDS • The employee must meet the physical demands needed to perform the duties of the written procedures. • Moving heavy molds and auxiliary equipment (>55 lbs.). • The employee is regularly required to stand, walk, talk, and hear.

WORK ENVIRONMENT The work environment is usually in an indoor concrete floored precision machining shop. General noise level can fluctuate from medium to high on a constant basis
