About the Department Our Post-Acute Care Transitions (PACT) Program is an award-winning initiative which focuses on patients who are medically stable and require nursing home level of care, but who do not yet have a safe discharge plan after hospitalization. Using a multidisciplinary approach, PACT coordinates a seamless transition for patients from the hospital to the nursing home where patients can access resources related to mental health, homeless medicine, substance use, wound care, housing programs, and palliative services. Once the patient is ready for discharge from the contracted nursing home, a team-based assessment informs a care coordination plan for patient integration back into the community. About the Position The Post-Acute Care Transitions Program is seeking a physician join our established team. You should have a current CA medical license, DEA and eligibility to participate in Medi-Cal/Medicare. The successful candidate will work closely with our robust interdisciplinary team to provide clinical care to our diverse, fragile patients, as well as coordinate transfers from the hospital, to the contracted SNF and facilitate safe discharges to the community. There is also opportunity for QI projects for the division as well as teaching Internal Medicine residents. This position also offers full time employment to work half time in Post-Acute Care Transitions and half time in Primary Care clinic. Performs a wide variety of diagnostic activities, methods, procedures and techniques to layout, install, align, repair, overhaul and maintain various types of machines, including mechanical, hydraulic, pneumatic, electrical systems and operations Applies knowledge of PLC, servo drives and servo motors errors Studies blueprints and manufacturer’s manuals to determine correct installation and operation of machinery Locates sources of problems by observing mechanical devices in operation, listening for problems and using precision measuring and testing instruments Fabricates small parts as needed to minimize downtime Executes on preventive maintenance schedules to ensure uptime/downtime targets are achieved Controls downtime by partnering with production and scheduling teams to exchange information on mechanical issues Ensures effective communication between shifts and teams by following documentation procedures and standards Ensures all maintenance activities are in compliance with federal, state and local laws andcodes, as well as company policies which govern safety, food safety Observes good manufacturing practices ·Understands and applies safety practices, and works to develop a basic understanding ofOSHA Ensures that safety is given top priority in work execution, including Arc/Shock Hazard safety, confined spaces, lockdowns, Hazardous materials, hard tools safety, etc. Partners with engineering team to align on operation of new machinery Works with broader engineering team to discuss machine operations, repairs, etc., toensure optimal maintenance schedules and outcomes Performs other duties as required

Physical Requirements

Frequently requires standing, lifting, walking, reaching, stooping, bending, smelling, listening, speaking, and seeing to perform the essential job duties. Work conditions vary, working time will vary based upon work / project needs, and flexibility is critical. Temperatures range from 5 to 90 F degrees in processing departments. Exposure to potentially dangerous machinery and equipment are present in the plant. Potential electrical, mechanical, and chemical, hazards are also present. The noise level is high and constant. Floors are damp and slick. Must be able to bend, stoop, pull and lift up to 25 lbs. to stand for long periods of time Close visual acuity to view a computer terminal, analyze reports, manage data and detail
