Behavioral Health Care Manager

Job Description Job Description Job Description: The Behavioral Health Care Manager will be responsible for reviewing patient charts and communicating with the Clinical department and providers about any irregularities as part of chronic care management. Responsibilities:

Provides assessment and care management services, including: The administration of validated rating scales Initiates behavioral health care planning concerning behavioral or psychiatric health problems Revises / modifies care plans for patients not progressing or whose status changes Brief psychosocial interventions if needed Ongoing collaboration with the billing practitioner Maintenance of the registry/tracking sheet C Consultation with the psychiatric consultant Has a continuous relationship with the patient. Collaborative, integrated relationship with the rest of the care team Conducts an interdisciplinary care plan meeting to go over the patients beneficiary to the program

Qualifications:

Bachelor's degree in social work Experience in long-term care preferred Experience in behavioral health preferred
