Position Title: LPN – Home Health Assessor - Contingent Reports to: RN Supervisor
Position Summary: The LPN’s primary responsibility will be assessing new and existing personal care and homemaking consumers, evaluating the physical, psychosocial, spiritual, and environmental realms. This position works with clients individually to assess client needs and services upon completion of the assessment submits required documentation. The LPN will also be responsible for providing supervision to the Home Health Aide staff within the Home Care Program. The LPN will assist other department staff with activities such as client follow-up, field staff in-services and covering the scheduling of services as needed. Work performed in this position will be of a professional and confidential nature. Work requires an attention to detail, flexibility and the ability to handle multiple functions at one time. This position will have frequent contact with agency personnel and clients. A positive and cooperative image and attitude is crucial for this position. Work performed is under the general supervision of the RN Supervisor and in conjunction with other agency staff. This position requires a significant amount of independence, self-direction, and time management to accomplish its essential functions and responsibilities.
Principle Responsibilities/essential functions:
Responsible for conducting in-home admission assessments to determine the appropriate level of care, completing all necessary evaluations including nursing service reports, patient and nursing assessment forms, activities of daily living and instrumental activities of daily living forms, consent forms, financial responsibility forms, funding documents, home environment forms, home health aide and homemaker care plans. Responsible for developing service plans, providing resources, making referrals, reporting concerns, completing all required admission documents, and communicating with case managers as needed. Responsible for interviewing, hiring, and orienting home health aide staff according to to program and Agency policies and procedures. Responsible for providing guidance to field staff, following up on concerns and reporting concerns to supervisors. Responsible for completing re-assessments within a designated timeframe and communicating with case managers in regards to service changes as needed. Responsible for performing other department or agency functions as necessary. This includes attending and presenting in meetings, adult immunizations, chart audits, assist Help at Home Manager with reports and audits, answering department phones, and performing personal care.
Working Conditions: Work is primarily in agency office and in client homes. Work requires employee to drive in all types of weather conditions. Work is generally between 8:00am and 5:00pm. Evening and weekend assignments may occasionally be required. This position is contingent and hours will be offered, as they are available.
Qualifications: A current LPN license for the state of Ohio. Minimum of three years supervisory experience. Must possess a valid Ohio driver’s license, have a vehicle available for agency use and maintain adequate insurance. Maintain personal professional liability insurance in the amount of one million dollars. Employee must have the ability to lift 30 pounds.
Knowledge, Skills, and Abilities: General knowledge of and ability to apply public health nursing principles and practices General knowledge of pharmacology Possess a wide range of nursing skills and motor dexterity in relation to medical-surgical nursing procedures Ability accept responsibility and understand limitations professionally, legally, and ethically Ability to work with minimum supervision Ability to exercise professional judgement and make meaningful observations and evaluations Ability to develop and maintain effective working relationships with LifeCare Alliance staff, clients, and client caregivers, other community health and social services, and physicians Ability to adequately document pertinent information on appropriate client forms Completes and returns all appropriate documentation within the time period established by program policies and procedures.
