Essential Skills:
Excellent interpersonal communication skills Knowledge and understanding of social service programs and case management. Ability to build and maintain consistent contacts, promote participation and build morale with residents.
Responsibilities:
Conduct intake appointments upon resident move-in Meet with residents on a regular basis. Create and case manage resident service records. Enter data and outcomes into an electronic reporting portal. Build and maintain relationships with community service providers, property management and other team members.
Coordinate supports and promotes positive outcomes. Assist residents in accessing community resources. Facilitate resident meetings. Encourage residents to be proactive toward addressing social, behavioral, and physical needs, maintaining self-sufficiency and housing stability. Educate residents on service availability, application procedures, and residents rights and responsibilities, Facilitate-e volunteer support, educational and recreational opportunities with community providers. Other duties as assigned.
Qualifications:
A B.S. degree in a social services field is preferred, Applicants with of a minimum of five years’ experience in human/social services or a combination of experience and education will also be considered. An understanding of social service systems of care and accessing community resources is essential. Reliable transportation and willingness to travel to both required sites. Strong verbal and written communication skills needed. Experience with file documentation and case management required.
