Position Summary: The Program Coordinator (PC) will be based out of our Centerville office and is responsible for ensuring the delivery of high-quality services to the individuals we serve. This role involves coordinating all aspects of care, acting as a liaison between participants, their support groups, families, case managers, and caregivers. The Program Coordinator will also promote our services within the community, build relationships with Service and Support Administrators (SSA), enroll new participants, and ensure compliance with regulatory requirements across our Dayton, Franklin, and Cincinnati locations. The Program Coordinator will collaborate closely with the HR department to coordinate staffing and scheduling, manage participant files and documentation, and support the company’s involvement in community outreach and partnership-building activities.
Key Responsibilities:
Service Coordination:
Ensure the delivery of high-quality services to participants by coordinating between personnel, support teams, and caregivers. Serve as the main contact for participants, families, case managers, and caregivers, ensuring clear communication and advocacy for each individual’s needs. Build strong relationships with Service and Support Administrators (SSAs) and other community partners to promote 360 Total Care and enroll new participants. Attend intake and ISP (Individual Service Plan) meetings, and ensure that care plans are accurately entered into the company’s electronic systems. Regularly verify that all participant documentation is accurate and in compliance with DODD and Medicaid regulations.
Staff and Activity Coordination:
Collaborate with the HR Coordinator to manage staff schedules based on participant needs across Dayton, Franklin, and Cincinnati. Work with HR to address personnel requirements, including recruitment and staffing adjustments as necessary. Create and update activity schedules for participants, coordinating events and outings with staff to promote community integration. Act as a backup in situations where additional support is needed, ensuring smooth daily operations.
Compliance and Documentation:
Maintain all participant and employee files in accordance with HIPAA regulations, ensuring confidentiality and accuracy. Verify weekly documentation, audits, and service records to ensure compliance with regulatory standards. Ensure that all participant and staff documentation is up to date in electronic systems, including medical administration, service logs, and activity plans. Stay informed on changes to DODD, Medicaid, and other regulatory guidelines, implementing necessary updates in service delivery.
Community Engagement and Advocacy:
Advocate for participants by maintaining clear communication with families, guardians, and support groups. Promote 360 Total Care within the community by attending fairs, events, and developing relationships with other businesses and organizations. Look for opportunities to secure grants, sponsorships, and partnerships that can benefit the company and its participants.
Health and Safety:
Coordinate medical appointments, monitor participant health needs, and ensure appropriate medical support is provided when needed. Ensure all relevant systems (e.g., medical, financial, environmental) are monitored for participant safety and well-being. Review program data and progress, making adjustments to ISPs as necessary in collaboration with the support group. Manage and review unusual incidents (UI) and major unusual incidents (MUI), providing backup and after-hours support if needed.
Training and Development:
Ensure staff complete all required training, including CPR, First Aid, and medication administration, and maintain up-to-date certifications. Support staff development through regular performance reviews and feedback, ensuring they meet the needs of the participants.
Position Requirements:
Location: Based out of Centerville Office, with services provided to the Dayton, Franklin, and Cincinnati areas. Education: High school diploma or GED required; Associate’s or Bachelor’s degree preferred. Experience: At least one (1) year of experience providing direct support to individuals with developmental disabilities. Skills: Strong verbal and written communication skills, the ability to make independent decisions, and the capacity to work a flexible schedule. Technology: Basic understanding of Microsoft Office software, specifically Word and Excel. Certifications: Must hold a valid driver’s license and be able to maintain certifications in medication administration, CPR, and First Aid. Requirements: Position Requirements:
Education: High school diploma or GED required; Associate’s or Bachelor’s degree preferred in social work, health services, or related fields. Experience: Minimum of one (1) year of experience providing direct support to individuals with developmental disabilities, preferably in a supervisory or coordinator role. Experience with Individual Service Plans (ISP) and coordination of services for individuals with developmental disabilities. Familiarity with DODD, Medicaid, and HIPAA regulations preferred. Skills:

Strong interpersonal skills, with the ability to build relationships with participants, families, support teams, and community partners. Technical Skills: Proficiency with Microsoft Office Suite (Word, Excel, powerpoint, Outlook) and electronic documentation systems. Experience with Brittco or similar systems for managing participant and staff documentation is a plus. Certifications: Must hold a valid driver’s license and be able to maintain certifications in medication administration, CPR, and First Aid. Ability to adapt to changes in regulatory requirements and ensure compliance. Physical Requirements:
Ability to lift up to 25 lbs. as needed. Must be able to sit, stand, walk, and drive for extended periods. Flexibility to work occasional evenings or weekends as required.
