Job Requirements Must be AHA. Nursys verification within 30 days prior to start. For any disciplinary actions please complete FACIS Questionnaire available in EII document library and upload with board documents and verification. Current license and primary source verification within 30 days prior to start of assignment for all licensed professionals. Completed prior to start and updated every 3 years. HIPAA, Joint Commission -NPSG, OSHA Topics. Completed within a year of start updated yearly. Minimum 80% score. Only 2 attempts allowed. Group A is updated yearly, Group B-F is updated every 2 years. As of December 1st, 2023, a new edition was released. Traveler must complete with a passing score of at least 80% with only two attempts permitted. Agency to grade and upload with % on top of first page along with traveler’s e-signature and date. Extensions will be required to update to the latest version prior to clearance. Pre-Hire Modules required for Non-Allied Positions to be completed prior to clearance. Document can be found in the Document Library. 1st PPD within 1 year, 2nd PPD within 30 days of start. And/or Negative IGRA within 90 days of start date lab printout prior to start. TB Questionnaire must be updated annually. If positive TB record; Requires Proof of Chest X-Ray within last 1 year to be accompanied by TB questionnaire. Additional Details Documented proof of two MMR vaccines or Positive IGG Titer. No declinations accepted. If Negative titer, administer two doses of MMR vaccine, four weeks apart. Required for all travelers. Preferred to be completed prior to start, can be completed onsite. PAPR will not be completed on site. WI and MI require 3M 1860 Regular, 3M 1860 Small or 3M 1870. Updated annually. Statement of good health or completed physical exam prior to first assignment, updated annually thereafter Flu season is October 1, 2024 through March 31, 2025. Exemptions/Declinations are accepted, but candidate will be required to wear a mask at all times.
