This position is responsible for promoting the recovery of children and families participating in the Ohio Rise program through the supervision of direct service care coordinators in the following counties: Fulton, Henry, Williams, Putnam, Defiance, Paulding, Van Wert, and Mercer. Bachelor’s degree in social work, psychology, or related field (master’s degree preferred). LSW, LPC, LISW, or LPCC preferred. Minimum three years’ experience in children's behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field providing community-based services to children and youth, their families, or caregivers. Must be proficient in the use of computers and software. Possess a valid driver’s license, reliable transportation, be able to be insured through the agency commercial policy, and have personal auto insurance. Prior supervisory experience preferred.
Duties:
Implement the required model of care, provide individualized care based on client need, and link clients to needed services and supports. Ensure assigned staff participate in ongoing training and coaching to ensure consistency in delivering care coordination. Monitor completion of paperwork by assigned staff, including initial assessments, CANS assessments, care plans, and discharges.
