Administrative Functions: Maintain a good work rapport with inter-departmental personnel.
Assist in planning, developing, organizing and implementing departmental activities as directed.
Assist coordinating departmental service with other departments
Adhere to all WHV policies and procedures.
Perform administrative duties such as: filing, completing forms, reading memos and attend meetings, as directed.
Create and maintain a good customer service environment.
Training/In-Service Attendance:
Attend and participate in all required in-services and educational classes.
Attend and participate in workshops, seminars and coursework, as directed.
Safety and Sanitation:
Follow and establish safety procedures and precautions when performing the aspects of your job.
Report all injuries immediately to your supervisor.
Lift using proper technique. Assist others when directed.
Report all unsafe and hazardous conditions and equipment to your supervisor.
Maintain a work area that is clean and hazard free.
Follow all infection control procedures as they apply.
Know and follow fire and disaster plans.
Use personal protective equipment when performing tasks that might involve infectious materials.
Equipment and Supply Usage:
Report all equipment malfunctions or breakdowns immediately.
Report low or depleted supplies to the appropriate person.
Use equipment for its intended purpose.
Follow correct official procedures when using equipment.
Resident Rights:
Maintain the confidentiality of resident information.
Handle all resident property with respect.
Knock before entering into a residents room or dwelling.
Ensure that residents are treated fairly and with respect allowing for individuality and privacy.
Uphold the residents’ civil rights.
Regulatory Compliance:
Follow all OSHA, CDC guidelines.
Review book of Material Safety Data Sheets prior to working with any chemicals.
Treat all team members fairly and equally without regard to race, color, creed, national origin, age, sex, religion, handicap or marital status.
Adhere to all local, state or federal regulations as they apply.
Completes annual compliance training and exhibits behavior as set forth by the code of conduct in the performance of their duties.
Abides by White Horse Village’s code of conduct in the performance of their duties.
Home Care Competency:
Demonstrates competency in the following areas:
Maintains client confidentiality
Understands and promotes consumer control and the independent living philosophy
Assists residents with activities of daily living (as appropriate)
Recognizes and reports client changes

Understands and practices universal precautions
Understands White Horse Village protocols for handling emergencies
Completes required documentation
Understands departmental practices for reporting abuse and neglect
Understands departmental practices for dealing with difficult behaviors
Committee Function:
Serve on committees as assigned.
When serving on a committee, participate to the fullest of your ability.
Leadership Functions:
Provides resident care and services that support the White Horse Village Mission, Vision, Values and Health Care Philosophy.
Plan and develop companion schedules for RSSP.
Maintain master schedule for each companion’s caseload.
Maintain accurate records of absenteeism, lateness and notify the Social Services Manager of any attendance issue.
Verify timesheets and provide information to accounting for payroll and billing.
Complete referral form for all new requests for service.
Provide home visits to assess needs of residents requesting services.
Maintain referral log for all medical escort requests.
Assist with special projects.
Complete service plan for residents using RSSP.
Review service agreement with residents requesting RSSP services.
Adhere to established budget. Participate in the planning of yearly budget.
Order supplies as needed (e.g. timesheets, uniforms, etc…).
Perform all other duties as assigned.
Resident Care Functions:
Provide back-up coverage when regularly scheduled companion is not available.
Maintain small client caseload (approximately 25% of hours).
Provide assistance with Activities of Daily Living (ADLS), encouraging maximum independence: a. Dressing and undressing b. Bathing/showering c. Personal hygiene (shaving, dental hygiene, nail care, hair care, foot care) d. Mobility devices (cane, walker, wheelchair, crutches), monitors safe transfer and ambulation techniques e. Care of ADL devices such as eye glasses, contact lenses, and hearing aids f. Incontinence care Provide assistance with Instrumental Activities of Daily Living (IADLS): a. Telephone – assist with calls, hearing and vision adaptive devices b. Housekeeping tasks such as tidying room, dusting, bed making and ironing. c. Laundry d. Coordinates transportation e. Shopping, errands, medical appointments, etc. f. Cooking g. Making appointments
Provide reminders or cues to take medications.
Assist with health maintenance and monitoring tasks: a. Observe and report changes in residents’ physical condition, cognitive, and emotional status to supervisor or outpatient department as needed. b. Monitor for environmental safety hazards. c. Report to program coordinator or director of social services as appropriate.
Assist with dining: a. Obtain meals, assist with set-up, and assist with meal consumption.
Plan and complete job duties with minimal supervisory direction, including appropriate judgment.
Human Resources Functions:
Perform reference checks of applicants seeking positions to assure appropriate qualifications.
Maintain licenses and certifications of nursing staff and assure they are current.
Complete Status Change Form for any changes in status, position, rate of pay etc in the effective pay period.
Process payroll.
Distribute paychecks for the RSSP team member.
Rectify any paycheck discrepancies for RSSP team member.
Adhere to human resource policies and procedures as well as specific departmental policies and procedures related to leave requests, overtime usage etc when scheduling staff for work.
Responsible for interviewing prospective companions and other team member, as determined by the Social Services Manager.
Participate in recruitment efforts by attending Job Fairs etc.
